Intermittently a recovery point set (full + incrementals) will become out of sync. The following is an example:
Backup schedule Monday-Saturday 13:00
New base (set) created using custom weekly option - Sunday at 10:00
Set #23 is created on 19/01 and set #24 is created on 26/01. The next incremental backup after 26/01 is actually part of set #23 instead of set #24. See below image for details:

The root cause appears to be related to a crash (or hang) of the Symantec System Recovery service - see error section above for what you would expect to see in the event logs on the affected server.
