Long Term Disability

Once Short Term Disability (STD) has been exhausted, you may be approved for Long Term Disability (LTD) income replacement at 60% of your monthly target earnings (OTE) up to $20,000 per month.

This plan requires the appropriate approvals from Veritas' LTD vendor, Liberty (800) 320-7585, and your medical provider.

Long Term Disability Benefit

Once you have been continuously disabled for 180 calendar days and your claim for long term disability (LTD) is approved, the LTD Plan will pay 60% of your monthly base pay or on target earnings (OTE) up to$20,000 per month. Your benefit amount is offset by any other disability-related income you may receive, such as Social Security disability or Workers' Compensation pay.

You are not required to complete an LTD claim form. Liberty is also Veritas' Short Term Disability (STD) claim administrator and they will automatically submit your LTD claim to their LTD Administration Group about halfway through the required 180 day LTD waiting period.

Liberty will contact you in writing at least 30 days prior to the end of the 180 day waiting period to inform you of the status of your LTD claim.

Coverage continues and your contributions will be deducted from your benefit checks.
Health Care Flexible Spending Account Contributions cease.
Dependent Care Flexible Spending Account Contributions cease. Within 31 days after you return to work, you must re-enroll  in the Dependent Care Flexible Spending Account (FSA) if you wish continue your participation in this plan. If you fail to re-enroll, your annual Dependent Care FSA will be adjusted to reflect your year-to-date contribution. December 1 is the cutoff to make any changes to current benefit year.
Employee Assistance Program Coverage continues.

Holiday Pay Ineligible
Business Travel Accident Insurance Coverage ceases.
Long Term Care When you began STD, you would receive a bill from Prudential to continue coverage your own cost. Service continues while on LTD as long as you remit payment.  If payment is not received, coverage will lapse. You may re-enroll when you return to work.
Critical Illness Insurance
Pet, Auto and Home Insurance
When you began STD, you would receive a bill from MetLife to continue coverage at your own cost. Services will continue as long as you remit payment to MetLife.
Tuition Reimbursement Program Ineligible
ARAG Group Legal Plan Contributions and coverage cease after STD elimination period. You can continue coverage by contacting ARAG and paying for the plan directly.
401(k) plan Contributions and loan payments cease.
Employee Stock Purchase Plan (ESPP) Contributions cease during leave.


Leaving the Company

Eligibility for new Long Term Disability insurance coverage ends on your date of termination. You have the option to convert your existing coverage.
