What Do I Need to Know to Enroll?

Make sure that you and your dependents/beneficiaries meet eligibility requirements.

U.S. Veritas employees actively working at least twenty (20) hours per week are eligible to participate in Veritas' U.S. Benefit Programs.

If you are rehired by Veritas within 30 days from your previous termination date, your health benefits will be reinstated to your previous elections (unless you have experienced a qualified life event, in which case you may make applicable changes within 31 days of your rehire date). If you are rehired by Veritas after 30 days, you have 31 days from your rehire date to make your benefit elections.

Dependent Eligibility

Family members ("dependents") may be eligible for certain benefits, such as health care and life insurance. You are permitted to enroll or update your benefit elections in the following circumstances; when you are hired, during an Annual Open Enrollment event, or if you experience a qualified status change, such as a marriage, divorce or birth/adoption of a child. Eligible dependents include:

  • Spouse
    A spouse is a person of the same or opposite sex who is married to the employee (excluding separation and divorce).
  • Domestic Partner
    A domestic partner is a person of the same or opposite sex with whom you share a committed relationship as evidenced by a shared residence and record of financial interdependence. View Domestic Partner criteria.
  • Children up to age 26
    You can over your children up to age 26. Medical care for your dependent child(ren) ends on the last day of the month in which they turn 26.
  • Disabled children over 26 (in certain cases)
    You can cover your disabled child (at any age), provided he/she was previously covered under Veritas benefits before age 26.

Individuals not specifically listed here are not eligible for coverage, including those who may perform services for Veritas (such as temporary employees or contractors) and certain family members (such as parents and siblings). An ex-spouse is not an eligible dependent, even if you have a court order to provide health insurance coverage.

Enrolling ineligible dependents constitutes fraud and coverage will be rescinded.

Double Coverage/Maintenance of Benefits (MOB)

"Double coverage" applies to you if your dependents are covered under Veritas' medical and/or dental plans and other medical and/or dental plans. The Veritas medical and dental plans pay claims based on Maintenance of Benefits (MOB). Under MOB, the Veritas medical and dental plans are the secondary plan for your dependents who are also covered under other plans. If the benefits paid by the primary plan are less than the benefits normally payable under the Veritas medical or dental plan, then the Veritas plan will pay the difference between the two plans. If the primary plan payment exceeds the benefit normally payable under the Veritas medical or dental plan, as the secondary plan, Veritas would pay nothing.

Veritas Couples

If both you and your spouse or domestic partner are employed by Veritas, you may not have "double coverage" under the Veritas medical, dental, and vision plans. This means you may not be covered as both a Veritas employee and as a dependent of another Veritas employee for these plans. In addition, you and your spouse or domestic partner may not cover the same child as a dependent for any benefit.

Domestic Partner Eligibility

A domestic partner is someone of the same or opposite sex with whom you share a committed and mutually exclusive relationship (as evidenced by a shared residence and record of financial interdependence).

To be eligible for coverage, you and your partner must:
  • Be at least 18 years of age
  • Have lived together for a minimum of six months
  • Be in an exclusive, mutually committed relationship similar to that of marriage
  • Be financially responsible for each other's well being
  • Not be married to anyone else or have another domestic partner (any such past relationships must have been dissolved for at least six months)
  • Not be related by blood to any degree that would bar marriage in the state in which you reside
  For a Domestic Partner's child(ren) to be eligible as a dependent in Veritas' plans, the child must:
  • Be primarily dependent upon you, the employee, for support, and
  • Exist in a parent-child relationship with you, based on the following:
    1. The child must be under 26 years of age.
    2. You must assume responsibility and control, including any and all debts incurred by the child (for example, charges for health care service and supplies).
    3. You or your Domestic Partner must have a court-appointed legal relationship with the child (for example, guardianship, adoption), or your Domestic Partner must be the biological parent, or the step parent of the child.

Domestic Partner Verification Process
You will add your Domestic Partner on MyVeritasBenefits. You do not need to provide paperwork for review at the time of enrollment.

Imputed Income
Be aware that if you elect health care coverage for a qualified domestic partner and/or a partner's children, the value of the domestic partner coverage will be considered imputed income; the total dollar value of the premium (paid for by Veritas and you) will be considered taxable income and reflected on your annual W-2 each year
